The Ground Electrode in EMG: Placement, Types & Common Mistakes

The ground (or reference) electrode doesn't record the signal you're actually interested in, which is exactly why it's the electrode most likely to be placed carelessly — and why poor grounding is one of the most common, most fixable causes of a noisy EMG trace.

What the Ground Electrode Actually Does

Every EMG or EEG recording measures a voltage difference between electrodes. The ground electrode gives the amplifier a stable, neutral reference point to measure that difference against. Without a good ground, the amplifier has no clean baseline, and common-mode electrical noise — from nearby equipment, room wiring, even the patient's own body — shows up in the trace instead of being cancelled out.

Where to Place It

General placement principles that apply across most EMG setups:

  • Keep it away from the recording and stimulating electrodes — too close, and it can pick up the same signal you're trying to isolate, reducing the noise-cancelling effect.
  • Use electrically neutral tissue — bony prominences with minimal muscle underneath (wrist, ankle, olecranon) are standard choices precisely because they generate little electrical activity of their own.
  • Prep the site the same as any other electrode — skin oils and dead skin raise impedance at the ground just as much as anywhere else. A ground electrode with poor skin contact is a common, overlooked source of noise.

Common Mistakes

  • Skipping it under time pressure. A "quick" study without a properly placed ground is the fastest way to end up repeating it.
