Why Gauge and Length Matter

Choosing the wrong needle electrode size can mean a painful insertion, a poor signal, or simply not reaching the target muscle. Needle electrodes are sized two ways: gauge (the needle's diameter - higher number means a thinner needle) and length (how far the needle reaches).

Gauge: Thinner Isn't Always Better

Gauge Relative Thickness Typical Use
23-25ga Heavier Deep muscles where rigidity helps insertion (paraspinals, gluteal)
26-27ga Standard Most limb muscles - balance of comfort and control
28-30ga Finer Facial muscles, pediatric patients, pain-sensitive individuals

Length: Match It to the Target Muscle Depth

20-25mm (0.8-1.0") Superficial muscles - facial muscles, first dorsal interosseous
37-50mm (1.5-2.0") Mid-depth muscles - biceps, triceps, gastrocnemius
75mm (3.0") Deep muscles - paraspinals, iliopsoas, gluteus medius
100mm+ (4.0"+) Very deep targets or larger patients

When in doubt, a longer needle is usually the safer choice - a needle that's too short simply can't reach the target, while a longer one just requires careful depth control during insertion.
